PLAGUE
It is basically and primarily a zoonosis disease caused by Y pestis involving rodents and fleas. It exists in natural foci and is transmitted by infected fleabites to human livings or intruding into the same ecological environment. DETECTION AND TREATMENT
Plague occurs in 3 forms :
A. Bubonic plague
B. Septicemia plague
C. Pneumonic plague
The incubation period varies from 1-7 days. Mode of spread – Human plague is most frequently contracted from : A. A bite of an infected fleas. B. A direct contact with the tissues of infected animal C. By droplet infections from cases of pneumonic plague.
The absolute confirmation of plague infection in human beings, rodents or fleas requires the isolation and identification plague bacillus Treatment : The drug of choice for the treatment of the plague is tetracycline. For adults the doze is 500 Mg for 6 hourly for 5 days. The cheaper alternative is sulfonamide 2-3 gms daily for 5—7 days.
PRECAUTION
Vaccination _ Immunization with plague vaccine is a valuable preventive measure. Vaccine is given subcutaneous in two doses of 0.5 & 1.0 ml at an interval of 7-14 days. Health Education is an important part of any plague control programme. Health Education should aim at providing the public with the facts about plague and enlisting their co-operation. Surveillance F The areas where natural plague foci exist or where there is history of past infection, surveillance is essential which h cover all aspects of rodents and human plague. Chemoprophylaxics is a valuable preventive measure highly recommended. It should be affected to all plague contacts medical nursing and public health personnel exposed to the risk of infection.
FACILITIES
The diagnostic facilities available at plague surveillance unit, NICD, Bangalore For surveillance activities there is one plague control unit at Kolar. (At the unit the surveillance activities have been standing still, due to abolish of mazdoors posts, the revival of this unit is under process).
